2019 Registration form

Registration deadline is October 15 for travel teams and November 10 for house teams.

Cost:
Squirt A/AA = $1050   Squirt B = $950
Pee Wee A/AA = $1100   Pee Wee B = $975
Bantam A/AA = $1150   Bantam B = $1000
Midget A = $1250

> Submit Official Roster via mail or email
> Submit payment via check payable to Mid-Michigan Silver Stick
> Make hotel reservations via hotel tab (teams are required to stay at the Tournament selected hotels)

Cancellation Policy: Any team that cancels 30 days prior to their tournament date will be refunded half of the entry fee. Less than 30 day cancellation, team will forfeit complete entry fee.
