Here are the forms to use for regional tournament registration. If you submit your registration form online through the MidAm Registration Form Tab, you will still need to submit a Team Roster Form to the tournament director, Jennifer DeGroote. It can be sent with your payment (made out to Fort Wayne Silver Stick) to: Jennifer DeGroote, 16118 Canyon Run, Fort Wayne, IN 46845. The team roster form can also be scanned and emailed to:
