Parkhill Regional Silver Stick Registration Form, Parkhill Regional, Southern Region, 2015-2016, Canadian Regionals (International Silver Stick)

The Parkhill Regional Silver Stick Tournament runs from December 27th to December 30th, 2015 and hosts Novice A/BB, B/CC and C/DD.



Fees are as follows:

$750
Gate Fee $5/day

All teams will be required to produce an approved Hockey Canada, OMHA or Alliance roster indicating your team's division.  All teams will be grouped according to OMHA classification.

Please be reminded Silver Stick is a Regional Tournament and your team should register according to the boundaries set forth from International Silver Stick. If you are seeking to attend a tournament outside your assigned Region silver Stick Centre, you will require a release from that Centre.

 Payment Options and Tournament  Acceptance

1. Cheque – After you Submit Form exit the screen and send in a cheque payment. Please note your  team name, level and division on the Cheque

 Make Cheque payable to North Middlesex and District Minor Hockey Association

All cheques will be cashed on receipt. Cheques can be post-dated to October 1, 2015. NSF cheques will be subject to $50.00 administrative fee. 

Confirmation of Acceptance:

Your payment is  NOT your Confirmation of Acceptance.  All teams will receive a separate and distinct email “Confirming Acceptance” into the tournament.

Once you have received Acceptance email  - Teams MUST provide a Hockey Canada Roster & Travel Permit .  ONLY Players and Team Officials on approved Hockey Canada Rosters will be permitted to participate. No acceptations will be made.
